(Place a full stop after Mrs/Dr/Ms/Mr) Jane Lapaglia, aged 71 I am writing to refer Mrs Lapaglia, who will be under your care following discharge. Mrs . (Place a full stop after Mrs/Dr/Ms/Mr) Lapaglia lives with her husband, Joe, in a four bedroom (four-bedroom) unit. She has one son, Andrew, who visits them twice in (in is not needed, delete it) a week. She likes classical music, ballet and AFL. During hospitalisation, the patien (the patient – spelling miastake) was comatose and ventilated for 7days (7 days-leave speace between 7 and days) in ICU. In addition, she was dialysed for 3 months (weeks-3/52 means 3 weeks) , as a result , (place a comma after every linking phrase) her urea and creatinine levels were decreased and (however) they are stable at the (the is not needed, “at present” is good) present.
