Chapter 5: Health Promotion for Women Community Based Nursing Care Counsel on menstrual period: o Length: between 24-38 days. o Length of flow may be 3-7 days. o Do NOT want to douche after you become prego or after sex. o Wipe front to back and pee after sex. Vaginas are typically odorless; they have odor with bacteria and sweating. If the tampon stays in too long it could cause TSS (Toxic shock syndrome). o Wash hands before inserting tampons because don’t want to introduce bacteria. Dysmenorrhea (pg. 67) o Is a painful period. o Occurs at, or a day before, the onset of menstruation and disappears by the end of menses. o Primary: Defined as cramps without underlying diseases. They increase uterine contractility and decrease uterine artery blood flow, causing ischemia. End result is the painful feeling of cramps. Treatment: Regular exercise, rest, application of heat, and good nutrition. Avoid salts, caffeine, and chocolate. Take Vitamin B6 Helps relieve bloating and irritability. Take vitamin E decreased cramps and breast tenderness. o Secondary: Is associated with pathology of their productive tract and usually appears after menstruation has been established. Could be from endometriosis or PID. Hysterectomy may be tx of choice. Primary and secondary can coexist so an accurate diagnosis is important for proper tx Premenstrual syndrome (PMS) o Stress, Trauma, Obesity could predispose you to PMS. o Magnesium decreases fluid retention. o Calcium helps with bloating. o Can take Vitamin B6 (can decrease depression) and E (can decrease cramps and breast tenderness.) Fertility Awareness Methods Fertility awareness – based (FAB) methods/Natural family planning include methods that require a woman to monitor her fertile window and abstain from sex or use a barrier method during this time. A woman is most fertile from 3 – 6 days before ovulation until 1-day post – ovulation. FAB Methods require extensive initial counseling and are best suited for women with regular periods.

How to keep from being pregnant (FAB methods) o Rhythm Method. NEED TO KNOW YOUR CYCLE!!! If they are not regular, this will to work. o Basal body temperature method: Take temp for 3-4 months in the morning before any activity. Temperature will drop just before ovulation and almost always rise and remains elevated for several days after. During ovulation it will rise ½ to 1 degree. Because the temperature rise does not occur until after ovulation, a woman who had sex just before the rise is at risk for pregnancy.
