CNS DRUGS Generic: Adderall/ dextroamphetamine sulfate Trade: Procentra, dexedrne Classification: therapeutic class: CNS stimulants pharmacologic class: amphetamines Route, Dose, & Freq: 5 mg P.O. once daily or bid. Increase by 5 mgat weekly intervals, as needed. Rarely necessary to exceed 40 mg/day P.O. Onset unknown peak 3 hr duration 4-6 hr P.O. E.R. Onset unknown peak 8 hr duration 8 hr Action: Probably promotes nerve impulse transmission by releasing storeddopamine and norepinephrine from nerve terminals in the brain. Main sites of activity appear to be in the cerebral cortex and the reticular activating system Adverse effects: CNS: insomnia, nervousnesss, restlessness,euphoria CV: tachycardia, palpitations, arrhythmias GI: dry mouth, taste perversion, diarrhea, GU: erectile dysfunction META: weight loss MUSCLO: rhabdomyolysis SKIN: urticarial OTHER: increased libido Nursing Responsibilities: >blood pressure and vitals >relevant labs - may increase corticosterioid level >administration -avoid late evening doses, particularly with ER capsules, due to resulting insomnia >patient education -instruct patient to immediately report chest pain, shortness of breath or fainting -tell patient he/she may get tired as drug effects wear off >drug-drug -acetazolamde, adrenergic blockers, antihypertensives Indications for client rationale Patient may be taking medication for ADHD or narcolepsy Generic: Alprazolam Trade: Xanax, Alprazolan intensol, apo-alpraz, novo-alprazol, Classification: therapeutic class: Anxiolytic pharmacologic class: Benzodiazepine Route, Dose, & Freq: Anxiety. Adults: Usual starting dose is 0.25 to 0.5 mg P.O. t.i.d. Increase dose p.r.n. q 3 to 4 days. Maximum total daily dose is 4 mg in divided doses. P.O. Onset 15-30 min peak 1-2 hr duration unknown Action: Alprazolam depresses the CNS at the limbic and subcortical levels of the brain. It produces an antianxiety effect by enhancing the effect of the neurotransmitter gamma- aminobutyric acid on its receptor in the ascending reticular activating system, which increases inhibition and blocks both cortical and limbic arousal. Adverse effects: CNS: drowsiness, light- headedness, minor changes in EEG patterns, headache, confusion, tremor, dizziness, syncope, depression, insomnia, nervousness. CV: hypotension, tachycardia. EENT: blurred vision, nasal congestion. GI: dry mouth, nausea, vomiting, diarrhea, constipation. Hepatic: liver impairment. Metabolic: weight gain or loss. Musculoskeletal: muscle rigidity. Skin: dermatitis. Nursing Responsibilities: >blood pressure and vitals >relevant labs -May increase ALT and AST levels. >administration - Anxiety with depression also responds to alprazolam, but patient may need more frequent dosing. -Monitor patients during prolonged therapy with high doses because they should be weaned from the drug gradually to prevent withdrawal symptoms >patient education -Make sure patient understands potential for physical and psychological dependence with long-term use of alprazolam.
