CASE CHART 2 SOAP NOTE CASE:DATE:TIME: SUBJECTIVE ANALYSIS Case reports having the following symptoms during the previous two days: Raised rate of urination A scorching feeling Pain during urination The case has also been subject to these indicators recently Rise in lower duodenal ache Vaginal release History of ailments- The case reports the urinary symptoms alike to those caused by a urinary tract infection (UTIs) that commenced less than three years back.
