Palliative Program Have you had a family member recently diagnosed with an illness that could lead to debilitation? Do they have chronic obstructive pulmonary disease, congestive heart failure, or cancer? Do they frequent the hospital or emergency room for symptoms? If you answered yes to any of these, please keep reading, palliative care is the perfect option for them. Tell us about a healthcare program, within your practice. What are the costs and projected outcomes of this program? Palliative care is a symptom management program. The palliative team meets with a patient as soon as they are diagnosed with a non-curable disease or as soon as they are experiencing frequent hospitalizations. The palliative program focuses on quality of life vs. quantity. Once they meet with the patient they discuss what to expect from the disease and then the patient determines what is important to them in life and what their goals are during their lifetime. They also determine if they wish to have symptoms managed at home through medicine or if they wish to continue going to the hospital. Obviously, their initial plan may change throughout their disease process. In the beginning if hospitalizations are few and far between they may chose to continue being hospitalized, however as their disease progresses they may decide they no longer wish to be hospitalized, so the team will then do symptom management with medications at their home. Palliative care is all about communication with the patient. Palliative care is covered by Medicare and most insurance companies. It is often offered at no cost to the patient.
