John’s current mental state and well-being reflexes that he may be suffering from borderline depression and anxiety symptoms and is in need for clinical intervention. The referral process began this morning when he was brought here by his mother from home. John’s mother stated that he had a mental breakdown after eating breakfast this morning and feels in a bad way, that he has a sense of worthlessness and sad. John’s father had a mental outburst in the home thismorning which trigger John depression. John’s parents are threatening to get a divorce after a twenty-years of being married if the father does not receive professional help for his mental health issue. John states all this bother him and causes him not to be able to sleep at night. John
Psychological Assessment Report 3has a feeling of worrying and being depression over not having a job to help his parent and statesthat a job would help him with his depression and give him a sense of self-worth and pride. Afterinvestigating and verifying this information from both John’s parents and medical report history, I found this information to be true, creditable and reliable. “Psychological assessment contributesimportant information to the understanding of individual characteristics and capabilities, through the collection, integration, and interpretation of information about an individual (Groth-Marnat, 2009; Weiner, 2003).” SOURCES OF INFORMATIONJohn Lee is 33-year-old, Caucasian, single man who currently resides with his mother. The clientis in his first year of college where he is working on a bachelor’s degree in human resource at Ashford University. John is presently unemployed and have a desire to work part time while attending college. He suggested that he wanted to work at his university through a work study program. He is expected to start his new position at his school within the next two weeks. A year ago, John was admitted to Jonestown Mental Health Center where he reported that he felt severally depression and anxiety symptoms. John’s medical report states that his father, a Navy veteran show to have some mental health issues from servicing in a war zone. (Costa & McCrae, 1991) acknowledged that, "The standard questionnaire measure of the Five Factor Model (FFM),provides a systematic assessment of emotional, interpersonal, experiential, attitudinal, and motivational styles a detailed personality description that can be a valuable resource for a variety
