WAIVER OF CLAIMS
To Whom It May Concern:
This CERTIFIES that the undersigned student, with the conformity of
his
/her parents
/guardian, hereby waives the right to sue or file a claim against the
Ateneo de Davao University and ________________________________, the
firm or
establishment where I am presently undergoing On-The-Job Training
(OJT) under the Practicum Program, their respective officers, employers and
agents with respect to the following:
1.
Wages and other benefits under the Labor Code of the Philippines, as
amended, and
2.
Injuries sustained or death due to my acts/omission and not due to the
willful and malicious acts/omission of the institutions or respective
officers, employees or agents.
However, subject to the above-mentioned items, this shall not be
considered as a waiver of claims against the said institutions or their respective
officers, employees and agents for any and all criminal or malicious,
