COUNSELOR:
Now that you have taken the time to fill out and sign all of the intake forms, do you
understand them?
CLIENT:
Yes
COUNSELOR:
Also, it is very important that you understand that I am a mandated reporter, meaning that
I do have to report any intentions you may have to harm yourself or harm others, and any
suspicion of child or elder abuse. Other than that, everything we speak about from here forward
is covered under a very strict confidentiality agreement. Do you fully understand that
confidentiality and exceptions that have been explained?
CLIENT:
Yes, I understand.
COUNSELOR:
Great!
Because this is our first session together I would like to ask you some questions.
This helps me get to know you and understand your needs so I can better assist you. Does that
sound ok?
CLIENT:
Yes, I understand.
COUNSELOR:
Perfect, what or who encouraged you to come and see a counselor?
CLIENT:

I have had a few friends and family go to a counselor, in order to work through some
issues and I thought I’d give it a try, nothing else has been working.
COUNSELOR:
Is that what prompted you to come to seek treatment now? What are you hoping to gain
from these sessions?
CLIENT:
Things just aren’t going well. I think I am depressed and I think I drink too much. I just
want to feel happier. That’s what you do right?
COUNSELOR:
I can’t fix anything for you but I can help you to try and fix them yourself. So you say
you are feeling like you depressed. What symptoms are you experiencing that make you think
this? Can you describe them to me and when did they first appear?
CLIENT:
I want to and I do sleep all the time. What worries me the most is I don’t find anything
exciting anymore. So I never want to go out then I just end up drinking… a lot.
Sometimes I
don’t remember somethings, like when I went to bed or how I got there.
COUNSELOR:
Do you have anything in your life right now that may be causing you stress?
CLIENT:
I don’t… I guess the only thing would be causing me stress is my job laid me off.
COUNSELOR:
I can see how that would cause you stress. Thank you for answering all my questions so
far, it been very helpful. Now I would like to ask you about your Mental Health History, because

of how you have been feeling lately.
If you don’t know how to answer or at any point you want
to stop with the question, please let me know. Does that sound ok?
CLIENT:
Yes, ok.
COUNSELOR:
Have you felt depressed in the past?
CLIENT:
I saw my doctor as a teen cause I was feeling down all the time and she prescribed me
something for it.
I haven’t taken it in years…. That the only thing I can think of.
