Endodontic retreatment Techniques, Instrument design, Removal of filling materials, Broken Files and Posts Supervised by : Prof.Dr. Amr Abd Allah Prof.Dr. Nayra Mokhles
OBJECTIVES INTRODUCTION DIFFERENT RETREATMENT TECHNIQUES REMOVAL OF BROKEN FILES. REMOVAL OF DIFFERENT POSTS. REMOVAL OF DIFFERENT FILLING MATERIALS. HOW DOES RESTORATION OF ENDODONTICALLY TREATED TOOTH PLAY ROLE ?
Introduction The vast increase in Endodontics can be described as the “ good news- bad news dilemma ” . The “good” news is that hundreds of millions of teeth are saved through the combination of endodontics, periodontics, and restorative dentistry. The “bad” news is that tens of millions of endodontically treated teeth are failing each year for a variety of reasons. Therefore, the future of endodontics will include dealing with the treatment of its failures. Causes of failure of endodntic treatment : 1-Apical percolation. 2-Root perforation. 3- Unfilled canals. 4-Coexisting periodontal lesions. 5- Gross over or under extension of filling materials. 6- Coronal leakage due to loss of a restoration or recurrent decay may also contribute to endodontic failure . Madison S, Swanson K, Chiles S. Comparison of CoronalMicroleakage in endodontically treated teeth obturated with gutta percha and various sealers. J Dent Res 1986 ; 65 : 298.
Failures can be treated by : 1- Non-surgical endodontic retreatment . 2- Surgical endodontic retreatment. 3-Extraction and place an implant . Ralan Wong . Conventional endodontic failure andretreatment.Dent Clin N Am 2004 ; 48 : 265-289.
Factors affecting the choice of retreatment options : 1- Amount of remaining tooth structure . 2- Restorability of tooth . 3- Quality of previous restoration. 4- State of host 5-Location of infection . 6- Financial consideration . The retreatment of previously treated root canals is a common procedure in endodontics successful retreatment requires the effective elimination of necrotic tissue and microorganism -infected materials, such as gutta-percha and sealer. Retreatment involves removing the previous root canal filling and performing advanced shaping, cleaning, and filling operations. Hulsmann M, Stotz S. Efficacy, cleaning ability and safety of different devices for gutta percha removal in root canal retreatment. Int Endod J. 1997 Jul;30(4):227-33
Root canal fillings can be removed with solvents, heating apparatuses, lasers, hand or rotary files, or ultrasonic instruments.
