Application Form for: GAVI Alliance Health System Strengthening (HSS) Support 2009 application deadlines: 1st May and 11 th September An electronic version of this application form is available on the GAVI Alliance website ( ) . Please send this electronic application with attachments (including the signatures page) to the following email address: [email protected] Enquiries to: proposals or representatives of a GAVI partner agency. The Proposal and attachments must be submitted in English or French Please ensure that the application has been received by the GAVI Secretariat on or before the day of the deadline. Proposals received after that date will first be taken into consideration for following review. GAVI will not be responsible for delays or non-delivery of proposals by courier services. The GAVI Secretariat is unable to return submitted documents and attachments to countries. Unless otherwise specified, documents will be shared with the GAVI Alliance partners and the general public.
