Interdisciplinary Team Reflection Communication is very important within any medical facility. Discussion with the interdisciplinary team is what produces a plan for the patient. Our patient is a vegan and is starting on coumadin. Vitamin K is the antidote to coumadin. Vegans eat a lot of green vegetables and that is where vitamin K is prevalent. This patient needs to discuss with the dietician why it is important to watch her vitamin K consumption and keep it similar every day. Since getting in with a dietician would probably take a few days, the nurse gave the patient information on what foods are high in vitamin K and why it is important to keep it the consumption amount the same every day. The patient is also taking ibuprofen 400-600 milligrams 2-3 times per day for pain relief for her knee. This medication is contraindicated for coumadin because it also thins the blood. The nurse would want to educate the patient on this contraindication and give her some other options for pain relief, such as Tylenol.
