Ped 0.1-2ml every 2-5min, titrate to blood pressure. Epinephrine (1:1,000) Adult 2-10 mcg/min IV/IO infusion for hypoperfusion. Titrate to maintain SBP>100mmHg. Ped 0.1-2mcg/kg/min IV/IO infusion for hypoperfusion. Titrate to maintain a SBP>70 + (age in years x 2) mmHg.

Atropine Adult 0.5mg IV/IO, repeat as needed every 3 minutes, max dose of 3mg. Ped 0.02mg/kg IV/IO. 0.1mg, min single dose. 0.5mg,max single dose. Repeat every 3-5 min as needed until max 1mg for child and 2mg for adolescents. Epinephrine Pressor 1:100,000 (10mcg/ml) Adult 0.5-2ml every 2-5min titrate to blood pressure. Ped 0.1-2ml every 2-5min, titrate to blood pressure. Epinephrine (1:1,000) Adult 2-10 mcg/min IV/IO infusion for hypoperfusion. Titrate to maintain SBP>100mmHg. Ped 0.1-2mcg/kg/min IV/IO infusion for hypoperfusion. Titrate to maintain a SBP>70 + (age in years x 2) mmHg. Norepinephrine Adult 1mcg/min IV/IO for shock. Titrate up to 30mcg/min to maintain SBP>100mmHg.

Epinephrine Adult 1mg IV/IO push every 3-5 min as long as pt remains pulseless. 1:10,000 Begin 1000cc IV NS Bolus. Ped 0.01mg/kg IV/IO push every 3-5 min as long as pt remains pulseless. Begin 20cc/kg IV NS Bolus and repeat if needed to max of 60cc/kg. Choose either Amiodarone or lidocaine NOT both Amiodarone Adult 300mg IV/IO first dose, 150mg IV/IO second dose. Ped 5mg/kg IV/IO, may repeat up to 2 times. Do not exceed 300mg. Lidocaine Adult 1-1.5mg/kg IV/IO push or one time dose of 1.5mg/kg. May repeat every 3-5 min up to 3mg/kg. Ped 1mg/kg IV/IO/ET push. May repeat every 3-5 min up to 3mg/kg. Magnesium Adult 2gm IV over 2 minutes for torsades de pointes. Ped 25-50mg/kg IV/IO for torsades de pointes. Maximum 2 grams.

Baby Aspirin Adult 324mg (4 tablets) PO if patient is >18 yrs old and no known aspirin allergies. Peds Not indicated Nitroglecerin Adult 0.4mg repeated every 5 minutes. Up to 3 doses, as long as symptoms persist and SBP>100mmHg . Normal Saline Adult 500ml IV over 30 minutes unless signs of CHF. Peds Not indicated
