In the final report released by the Bureau of Ocean Energy Management,
Regulation, and Enforcement. was concluded that the disaster was a result of
BP’s poor decision-making that led to the explosion.
The investigation reported
that this disaster took place due to poor risk management, last-minute changes
to plans, failure to observe and respond to critical indicators, inadequate well
control response and insufficient emergency bridge response training for the
operation of the Deepwater Horizon (Poor Risk, 2011).
