The Veteran population consists of prior military personnel who are age 18 and older. Veterans come to the HCC for many reasons, for this assignment I will focus on the specialty clinic. The specialty clinic offers care in general surgery, podiatry, wound care, orthopedics, pain clinic, cardiology, and pre-operative services. The drivers are increased patient satisfaction, improvedpatient outcomes, and improved nurse satisfaction. Assigning a nurse to assist with discharge instructions can relieve the burden of the staff. The discharge nurse would be responsible for
NURSING LEADERSHIP3spending additional time with the Veteran while educating them. They would provide handouts as well as give verbal instructions. Having improved patient satisfaction improved the Veterans Affairs (VA) strategic analytics for improvement and learning (SAIL) reports. The SAIL model is a tool used to measure, evaluate and benchmark quality and efficiency within the VA medical centers. Improved patient outcomes would also result in better discharge education. Veterans would get verbal and written instructions and information. The patient would be able to referencethe written instructions received at the clinic to answer questions that may come up once at home. This would also improve nurse satisfaction resulting in less post appoint phone calls asking about discharge instructions or questions that they may have.
