Topic 2 DQ 1 What are social determinants of health? Explain how social determinants of health contribute to the development of disease. Describe the fundamental idea that the communicable disease chain model is designed to represent. Give an example of the steps a nurse can take to break the link within the communicable disease chain According to Green, Social Determinants of Health (SDOH) are conditions of living, such as housing, socioeconomics, transportation needs, quality of education, that directly impact health and access to health care needs (Green, 2018). All of these elements affect a person’s ability to have and maintain good health, when these elements are not the best it tends to hinder a persons health. For example when a person lives in a neighborhood where public transportation is not easily accessible it may make it harder for a person to make it to doctors appointments which can cause them to be unhealthy and could lead to the development of disease.
