Primary PatientThe patient is a 39-year-old female who is obese but otherwise healthy in appearance whose name is Angela. Patient is well groomed, dressed appropriately for the current weather, cooperative, and has a pleasant affect. Angela is of American Indian and Irish descent. She is a former smoker having quit over 15 years ago. She drinks beer “on the weekends” on occasion and smokes marijuana “pretty regularly”. Angela has been pregnant twice in her life resulting in two live births both girls aged 17 and 13 years. Patient states she is currently being treated for uterine fibroids with Lo Loestrin Fe. This course of treatment keeps her discomfort at a minimum and controls her periods otherwise she has “really heavy flow”. Patient states she feelsthat she has clinical depression but has never been officially diagnosed nor sought treatment for same. Patient states that she feels “down” two to three days of the week. These periods of depression have an adverse effect on her life insomuch that when she is feeling this way she avoids going out or interacting with friends or family.
