The nurse’s co-workers stepped in to save the patient’s life after the nurse gave a double dose of blood pressure medication. This nurse not only became a champion for patient safety after her mistake, but she helped other nurses close to her become patient safety champions as well.Misplaced feeding tube leads to 11 day old baby’s death.Reflection: A newborn baby had a feeding tube placed by an experienced nurse. The parents noted that the baby was not acting normal and questioned the nurse regarding the proper placement of the feedingtube. Instead of checking the feeding tube placement properly, the nurse dismissed the parent’s concerns in a defensive manner, and the patient, baby Grant, subsequently died. The parent then became patient advocates and patient safety champions by sharing their story.Course Project Week 2We Make MistakesStaff and leaders of a hospital discuss medical errors, acknowledge that they exist, and provide a positiveenvironment for staff and patients. Their number one goal is to have zero medical mistakes. They are trying to reach this goal by accepting the fact that mistakes happen, providing staff with positive feedback for sharing when mistakes happen or almost happen, and providing a safe environment for staff and patients.
