According to the Center of Disease and Control, the most common cause of hospitalization in the older population (65 and above) is Congestive Heart Failure, accounting for roughly 5.3 million diagnosis in the United States. Currently 1 out of 3 adults have been diagnosed with CHF and 1 out 9 deaths are results of cardiovascular diseases (CDC, 2016). CHF has become the leading cause of readmission within 30 days of discharge. The presence of CHF is associated with the presence of weakness and comorbidities, resulting in a decline of quality of life, frequent visits in the emergency room and enormous healthcare expenditure. Common reasons for hospital readmission are delay symptom recognition, medication and regimen compliance, lack of knowledge and skills for self- management. Promotion of compliance through comprehensive education can contribute to

One possible reason for these frequent hospital visits is due to the patient not complying with the training regimens necessary to keep them healthy . The purpose of this review was to study what education techniques were used during the hospitalization and what other education revenues could be used to prevent readmission and non- compliance in patients, including outpatient. This paper will discuss a nursing practice problem, a PICOT state men men t, background of the studies, method of the studies and result of the studies. The PICOT statement, (P) In congestive heart failure patient ages 18-64 years (I) do multidisciplinary educational approach, compared to (C) educational approaches utilized in inpatient and outpatient settings(O) decrease the number of readmissions in 30 days (T) to be studied for 6 months Your paper is about the education associated with CHF and teaching the patients.
