According to the Center of Disease and Control,the most common cause of hospitalization in the older population (65 and above) is CongestiveHeart Failure, accounting for roughly 5.3 million diagnosis in the United States. Currently 1 outof 3 adults have been diagnosed with CHF and 1 out 9 deaths are results of cardiovasculardiseases (CDC, 2016). CHF has become the leading cause of readmission within 30 days ofdischarge. The presence of CHF is associated with the presence of weakness and comorbidities,resulting in a decline of quality of life, frequent visits in the emergency room and enormoushealthcare expenditure. Common reasons for hospital readmission aredelay symptomrecognition, medication and regimen compliance, lack of knowledge and skills for self-management. One possible reason for these frequent hospital visits is due tothe patient not complying with the training regimens necessary to keep them healthy.The purpose of this review was to study what education techniques were used during thehospitalization and what other education revenues could be used to prevent readmission and non-compliance in patients, including outpatient. This paper will discuss a nursing practice problem,a PICOT statemenment, background of the studies, method of the studies and result of thestudies.The PICOT statement, (P) In congestive heart failure patient ages 18-64 years (I) domultidisciplinary educational approach, compared to (C) educational approaches utilized ininpatient and outpatient settings(O) decrease the number of readmissions in 30 days (T) to bestudied for 6 monthsYour paper is about the education associated with CHF and teaching the patients.
