BBH 301 Spring 2009 From “Guidelines for Developing an Informed Consent Form for Social Science Research” from the Penn State ORP The Informed Consent Form must include the following elements (not a complete list): - All wording must be at an 8th grade reading level or below. Avoid highly technical terms, jargon, etc. - Investigator contact information to include office mailing address, email and phone number - Purpose of the study - Procedures to be followed - Discomforts or risks (if none are known, state as such) - Benefits of the study to participants and society - Duration/timeframe of participation
