N Information You’ll Need to Provide When Requesting Grade Reporting Services • The service you are requesting. • Your full legal name, home address, gender, date of birth, AP number, and social security number (if you provided it). • The full name of the exam for which you are requesting the service (e.g., English Literature and Composition, not English) and the year you took that exam. • A credit card number and expiration date, or a check or money order for the exact amount due. Make checks and money orders payable to AP Exams. • Your signature. • When requesting additional grade reports, include the name, city and state, and college code of the college you would like to receive your report. © 2007 The College Board.
