Health care status: vaccination history, diagnostic testing, current medications Previous medical/surgical history Current complaint: onset, duration, severity, any treatment/ response to said treatment

History taking tips Be neutral—don’t be judgemental. Phrase questions carefully ex. “Has Prince Foofoo’s water consumption changed recently?” vs. “Is Prince Foofoo drinking more water?” Be thorough
