Mailing Address:    Phone:                                                                                                 Email: Bachelor or Master’s degree:                                                                                                        GPA: Institution (must be regionally-accredited): Other post-secondary institutions attended: Cumulative GPA on all undergraduate work attempted: IMPORTANT NOTE  : Students choosing this option are fully aware that it is LICENSURE ONLY and thus  they have applied as a non-degree licensure student or as a degree seeking student through  undergraduate  admissions.  Students pursuing this course sequence as a non-degree licensure-only  student should understand that after taking 24 hours of credit, he/she must meet with an Undergraduate  Admissions Officer to receive advice and to determine the purpose of continuing in this category.  Students in this category also need to realize that they are restricted from registering for classes until the  first day of the semester.   It would probably be better for the student to enter as a degree-seeking  student through the undergraduate admissions .  Doing so does not bind the student to getting a  second degree – he/she can stop once he/she has gotten all the credits needed for licensure.  Being a
