Unformatted text preview: Print Form Submit by Email Evaluation of Group Participation
Directions: Class participation grade will be assigned using the scale below. At the end of the
semester, you will be asked to use it to rate each member of your group, including yourself.
Please assign a number between 0 and 4 for each criterion (0 = "Did not meet the expectations at
all" to 4 = "Meet the expectations completely"). Once a number is assigned to each section, the
total (maximum of 20 points/person) points will be calculated automatically. Please be as honest
as possible with your assessment. Justify the "grade" that you give with comments. If you
divided up the work, what part of the work did the group member do? No comments to back up
your "grade" will result in a 10% decrease in your grade (you gave yourself) and a 10%
increase in the grade you gave your group members.
Note. Please make sure the name of the person who is conducting the evaluation is listed FIRST. When
done, click on the submit by email button EITHER at the top or bottom of form. It will be sent to me
