Assess and document BP, heart rate and respiratory rate every 4 hours. 8. Keep I&O record.
• If indwelling catheter in place, notify M.D. if urine output is less than 30mL/hour.
• If patient is voiding, notify M.D. if urine output is less than 200 mL in 6 hours. 9.
10. Notify MD if signs of Magnesium toxicity are present: RR less than 12/min and/or non-responsive to
verbal stimulation and moderate tactile stimulus, and/or bradycardia (less than 60 bpm).
Notify MD if contractions increased to equal or greater than 6 /hours. 11. STAT Serum Magnesium level for signs of toxicity. Call physician with results.
