Osterhout 1 Med Surg Nurs 330 08 Oct 06 Critical Thinking Question 3 The nursing assessment for the patient with heart failure (HF) focuses on observing for effectiveness of therapy and for the patients ability to understand and implement self management strategies. Signs and symptoms of pulmonary and systemic fluid overload are recorded and reported immediately so the adjustments can be made in therapy. The nurse also explores the patient’s emotional response to HF. The nurse explores sleep disturbances, particularly sleep suddenly interrupted by shortness of breath. The nurse also asks about the number of pillows need to sleep, actives of daily living, and the activities that cause shortness of breath. The nurse also explores the patients understanding of HF, the self management stragieties and the desire to adhere to those strategies. The nurse helps patients to identify things that they have lost because of the diagnosis, their emotional responses to that loss and successful coping skills that they have used previously. For the physical examination lungs are ascultated to detect crackles and wheezes
