BENCHMARK- PROFESSIONAL CAPSTONE AND PRACTICUM REFLECTIVE JOURNAL Introduction Nurses can find themselves face to face with a complex patient, not because of the illness but it can be because of culture, background, socioeconomic or lifestyle. A nurse has to be able to be well-rounded and diverse because we as nurses get to deal with all kind a patient with different cultures and beliefs. We have to able to provide culturally competent care and safe and effective and evidence base care at the same time. A BSN prepared nurse can work and are expected to work in a variety of setting and different roles like leaders, managers, case managers, it is important to apply learning skills and experiences that can contribute to the safest and effective delivery of care to the most vulnerable. I have also been able to master the context of the healthcare system in which I provide my roles within the systemic structure of healthcare organization. New practice approach Week one during my practicum in the emergency department I was able to look at the care provided and the safety of the patient with new eyes. I was able to review the objective of my course and review it with my mentor and to see where improvement was needed, brought it up the attention of my director ad sat down and came up with ways to improve the organizations as a whole. Implementing what I’m learning during my RN-BSN course, focusing on research and evidence practice I was able to implement change and lead my team and unit to better understand evidence base and apply it to everyday practice. Intraprofessional collaboration Providing care to patient requires more than just one person, it takes a multidisciplinary team to care for a patient. So, nurse have to able to collaborates with different personalities and know how to communicate to each other in order to provide great care for their patient. The 2

BENCHMARK- PROFESSIONAL CAPSTONE AND PRACTICUM REFLECTIVE JOURNAL Institute of Medicine (IOM) report, “The Future of Nursing: Leading Change, Advancing Health , identifies interprofessional collaboration among health care providers as an essential part of improving the accessibility, quality, and value of health care in the United States”. Nurse have to be full partners with physicians and other healthcare team members for successful patient outcome. Week 2 of my practicum, I’ve learn that ideas for improvement did not just affect the nurses in my unit but also affect the physicians and the other interprofessional teams, as a matter of fact the whole organization had to involved so they can share the new processes that has been started in the emergency department that will affect patient movement from the ER to the unit, they all had to be involved in other for the patient to be safe and received the proper care needed.
