The Hospital Readmissions Reduction Program (HRRP) is a program that is run by Medicare and focuses on value. This program reduces payments to hospitals with high readmission rates. The Affordable Care Act started this program and it went into effect Oct 1 2012. The program actually looks at 6 specific diagnosis Heart attacks, chronic obstructive pulmonary disease, heart failure, pneumonia, and coronary artery bypass graft surgery and hip or knee replacements. If these patients are covered by Medicare and have an unplanned readmission within 30 days of the initial admission repayment is reduced. This program encourages hospitals to deliver quality care as well as good discharge care to avoid costly readmissions. When looking at the triple aim frame work, this program touches on all three. Although the program I feel is more directed towards experience of care that being the quality as well has per capita costs. TH
