Unformatted text preview: Rita 1 of 1 Rita is a 35-year-old unemployed single mother whose chief complaint is that a voice is commanding her to kill herself. She has a history of 18 previous psychiatric hospitalizations for a variety of frequently changing symptom presentations. Her diagnoses over the years have included Schizophrenia, Bipolar Disorder with Psychotic Features, Major Depression with Psychotic Features, Schizoaffective Disorder, Posttraumatic Stress Disorder, Panic Disorder, Anorexia Nervosa, Multiple Personality Disorder, and Borderline Personality Disorder, and borderline Personality Disorder. She also frequently engages in self-destructive behavior such as cutting her wrists, head banging, and abusing prescription opiate analgesics and alcohol. Rita lives with her parents and her 10-year-old son in a very tense situation. Her parents constantly fight and are threatening to separate after 40 years of marriage.
