Bibliography This bibliography lists some of the references that have been used in preparing the book, and on which some of the ideas were based. In general it is simply a bibliography and not a set of specific references, although some material has been drawn from the material in this list; where that is the case a specific reference is made in the text. There is not a bibliography for all chapters because the information is subsumed into the books relating to adjacent chapters.
