RATIONALE FOR THIS BOOK I once overheard a conversation between two graduate students who were completing a course in theories of counseling. “Well,” said the first one, “I know all about the theories of Freud and Adler and Rogers and Glasser, and I got an A on the exam, but I really don’t know what to say or do when I’m with a client.” “I feel the same,” said the second student. “Maybe we’ll learn what to do in the next class.” Understanding, as well as wise application of theories of counseling and psychotherapy, is essential to the effectiveness of counselors and therapists. Consequently, learning of this information must be meaningful and useful to students. That is the goal of this book. We focus in this book on systems, strategies, and skills of counseling and psychotherapy in ways that reflect current knowledge about effective treatment, promote intellectual understanding of theories and interventions, and also enhance readers’ skill development, confidence, and professional development. By acquiring sound clinical skills, along with a cognitive grasp of treatment approaches, students as well as practicing counselors and psychotherapists will be prepared to help and collaborate with their clients. TREATMENT IS EFFECTIVE Psychotherapy and counseling are effective! Study after study has conclusively demonstrated the healing power of therapy In his 1995 research, described as “the most extensive study of psychotherapy on record” (p. 969), Martin Seligman found that 87% of people who reported feeling “very poor” and 92% of those who said they felt “fairly poor” before therapy showed clear improvement by the end of treatment. In addition, this study found that improvement was long lasting and was reflected in changes in presenting concerns as well as improvement in occupational, social, and personal domains. Long- term treatment was generally superior to short-term treatment, especially when treatment was terminated prematurely due to limitations in insurance coverage. This effectiveness study is so important because it was a naturalistic and realistic study, providing information on how treatment is actually performed. This means that it continued until people were satisfied with their progress or left treatment prematurely because they were not getting the help they wanted, could no longer afford treatment, or encountered other reasons to leave treatment. Treatment methods and schedules were not artificially controlled but, rather, allowed for self-correction to improve treatment, encompassed a wide range of specific and integrated treatment approaches, and often addressed multiple concerns. Lambert and Cattani-Thompson’s ( 1996 ) review of the literature focused primarily on structured and planned efficacy studies but drew conclusions similar to those of Seligman. They stated, “Counseling is effective …. The effects of counseling seem to be relatively lasting. These effects are attained in relatively brief time periods, with the percentage of clients who show substantial improvement
