Oft conflicts posed as efforts 2 “protect cts” i.e., can’t pres meds b/c not MD – but others besides MDs prescribe Ability of profess psychos 2 practice as believe is best has & will cont 2 B under threat from # of sources Competition with Medicine Psychos given + privileges over yrs - right 2 do psychotherapy- (N past only psychiatrists did it) - hosp privileges - prescription privileges Recent yrs - + competition 4 mental hlth business thus, attempts 2 constrain prac of clin psych/other mental hlth professions Relations b/t psych & medicine R uneasy Defining and “Diagnosing” Mental Disorders If psycho pxs viewed as biolog illnesses, then should B tx by MDs N clinics/hosps w/medications – by this def, psychos would B unqualified 2 tx them 2

PROBLEM is that mental d/os R both phys & psycho Biological Model Reps challenge 2 profess psych b/c profess psych has long emphasized psycho , familial , & soc causes of d/o challenge is not only 2 psych’s theoret stance, BUT ALSO 2 right & competence 2 tx psycho pxs HAS NOT been demonstrated that medicine + effective than psychoth - ECT, a biologically based tx, was fd useful 2 tx severe dep 4 many yrs rarely used p developmt of antidep med b/c resulted N injuries/degree of brain impairmt b/c of improved techs is regaining popularity works better 4 + tx-resistant dep used until combo antideps/intensive psychoth can begin 2 wk Thus, reps battle b/t psychiatry & psychology over tx issues 3

DSM largely written by med docs hlth insur cos/mgd care providers require DSM dx all mental health profess must use it reinforces med def of psycho d/os Some psychos don’t believe N dx but can’t be reimbursed if don’t use DSM Professional Competence psychiatry still argues psychos R not competent 2 tx certain gps of people reason: say b/c not trained N med & pharmacology (say this despite fact that doctorate N clin/coun psych may take + yrs 2 complete than med school AND amt of training N tx/psychopath is prob greater than that 4 psychiatrists) Certification some consumers/hlth care cos demand certification of specialized therapists N future, may need both license & certification N specialty 4
