Physical Examination Gen: The patient is a WDWN man who is cooperative for the examination. Eye contact is poor. He appears anxious and tearful. Vital signs: BP 110/66, P68, RR18, T98. 6 ; Wt 175 lb Skin: Good turgor. No cyanosis. There are some scars and sores from trauma and self-injury. HEENT: PERRL. TMs were not seen clearly due to wax. Bilateral nasal obstructions are noted. Gums appears to be normal. He is edentulous. Pharynx slightly injected. Neck/LN: WNL Lungs: Heart: RRR, no murmurs Abdomen: Soft, no tenderness, no masses, no HSM Rectal: Negative; stool heme(-) Genitalia: Normal uncircumcised male. Both testicles descended. No pain or abnormality noted. No hernias. Ext: Evidence of a healed scar on the left knee. Good range of motion. Ambulatory without assistance but gait unsteady. Abrasions on left forearm and elbow Neuro: During this examination there were mild oral movements. No tongue tremors and no excessive salivation or rigidity of extremities noted. Labs:
