Wellness Inventory Health Science 1: Understanding Your Health Wellness Inventory Assignment: Live Well Questionnaire Purpose: Students assess their health status using a program on the Internet hosted by the University of Wisconsin. Students answer questions related to 10 dimensions of health status: including physical health, drugs and driving, emotional health, occupational health, and spiritual health. A printout provides the student with scores enabling him/her to critically evaluate areas where improvement in healthy lives styles can be made. Students use this information to write a contract which designates specific health behaviors they would like to change. Students submit the results to the instructor at the beginning and again at the end of the semester , using an anonymous code, so that aggregate scores can be computed and comparisons can be made.
