In your experience, where do risk management plans fall short? In my experience, I find that risk management plans has fallen short due the fact that the actual practice of assessing risk and establishing planned contingencies for those risks usually is not given considerable or noticable effort. At times managers and or executive officers tend to disavow the probrability of possibility of events occurring.

This leaves the organization to complete disaster as when the unexpected occurs, there is no assigned protocol to follow and chaos begins to absorb into the organizational culture and management process....
