Introduction This memo  is a failure analysis report on the space shuttle Challenger. It  exploded  on January  28, 1986 just 75 seconds  after being launched  from the  Kennedy  Space Center, killing all seven astronauts  aboard  and  temporarily  halting the US space program  (author, date of publication). The following  sections  of this report describe the events leading  up  to and  during  the failure, the  reasons behind  the failure, and  the lessons to be learned  from this failure. 2. Description The Challenger  Space Shuttle, at launch, consisted  of four components: the  shuttle itself, a large external fuel tank attached  to the belly of the shuttle and
