For patient name… How many fields? (e.g. first, last, middle) First name first or last? Middle initial only? How long is each field (how many characters?) Any numbers in the name or only alphabetic? What about hyphenated names? What about maiden names? Any suffix? (e.g. Jr., Sr., III) Are any of these fields mandatory or optional? In what sequence will these fields be sent? 7
