Name __________________________________ Date ____________ Subject ___________ To determine the consistency of your exercise and eating habits, it may be beneficial for you to evaluate your eating and exercise habits for a complete week. Simply use the weekly self-evaluation score sheet in Table 9-5 and score yourself each day. The procedures to follow have been given previously. Calculating Your Score To calculate your score for the week, total your score for each day. You then add the 7 daily totals and divide by 7 to determine your average score for the week. Interpretation of Scores To have the consistency you need to be successful at losing weight or simply to have good consistent eating and exercise habits, you need to average 70 or more points. This score is based on the premise that you exercise no more than 5 times each week. If you exercise 6 times, you need to average 75 points, or if you exercise all 7 days, your average needs to be 80 points. Table 9-5 Weight-Management/Exercise Program -- Weekly Scoresheet Rate yourself on the following 11 guidelines each day and record your score for each of them. At the end of each day, add up all scores to determine your daily total. Name:
