Patient Label RACE/REFERRAL SOURCE/SUMMARY: The Patient is a 46-year-old Caucasian Male who presents today for his intake assessment. The patient is married and has 2 Adult male children aged 24 and 30. The patient does not have grandchildren. The patient is employed as a service accountant at Walmart. He was referred by our community outreach team. Patient reports overall health is stable. He states he has been feeling depressed for the last month. He has history of depression. His immediate reason for seeking professional help is “because I can’t live this way no more” CHIEF COMPLAINT: _____________________________________________________________________________________________ HISTORY OF PRESENT ILLNESS: The patient reports that his mental state of mind has been on the decline for “for two weeks now”. The patient states that he has been having symptoms of apathy and asthenia for the past two days. Patient states “nothing seem to motivate me. I can’t even go do my own groceries”. The patient reports his lack of motivation is affecting his work, and family life, “my wife doesn’t even want to be around me” he says.
