The first cases is an initial consultation since it doesn't provide a specific location I am going to assume that it is in a office or an outpatient situation. Since this is an initial consultation that would lead me to believe that the patient is a new patient and so the E/M codes 99205 because there was more than one body system that had to be reviewed by the doctor. The second case would lead me to believe that this is an established patient that is going to need minimal diagnosis. For this encounter I would use E/M code 99211 since this patient sounds like they are already an established patient. The third case is an established patient who is just coming in for routine blood work. For this I would use E/M code 99212 since both the history and examination are focused on one issue and the medical decision making is

Since this case is going to require examination of more than one body system E/M code 99214 would be used.
