B ONE MARROW CLINIC HISTORY & PHYSICAL EXAMINATION Patient Name: Quentin Asher Johnson Patient ID: 17-8921 Date of admission: 10/07 DOB: Age: 36 Sex: M Chief complaint: the patient is a 36-year-old black male with refractory Hodgkin’ s lymphoma who is admitted today with progressive hypoxia. HISTORY OF PRESENT ILLNESS: patient has known has Hodgkin lymphoma. In addition, he has bilateral pleural effusion left fusion large, right fusion small. His left fusion is known to be a quality effusion. This was taped several weeks ago by Dr. Altman at the pulmonary clinic. It has really accumulated and has been stable over the past few week. In addition, on Sep.20. he has a small right pulmonary embolism. He has been on Lovenox anticoagulation because pulmonary. His pulmonary embolism resulted in oxygen requirement. 2 days ago, in clinic, his oxygen saturation at rest, was 95% on room air. Yesterday patient presented to the clinic and was saturating 70th 2 liters with injection. He denied having increased symptoms of shortness of breath. CT angiogram was obtained which was unable to compared with previous CT. however is was reviewed by pulmonary staff who felt the primary fusion. As well as his lymphoma symptoms was stable inside. He demonstrated no new PE. No areas of consolidation suggested pneumonia. However, he just
