IDENTIFYING INFORMATION: Patient is a 40-year-old white married male who has two children
from a previous marriage.
CHIEF COMPLANT: “the alcohol brought me here.”
HISTORY OF PRESENT ILLNESS: According to Information obtained from the chart review, the
patient was brought in with an affidavit stating he has a drinking problem. “ It would be better off
for everyone if I wasn't here.” Patient states he was thinking about “down in my whole bottle of
Xanax.” The patient reported that he has a long-standing history of drinking and had treatments in
the past, but the longest variety he had was a year ago. He reported he has been drinking almost two
pints of whiskey per day lately. The patient reported he has a history of delirium tremens but denies
any history of withdrawals or seizures. He has tried to cut down but has been unable to. The patient
