The patient is seen in the office today for an orthopedic consultation of his
cervical region at the request of Wanda P. Dale, MD.
HISTORY OF PRESSENT ILLNESS: This 49-year-old Caucasian gentleman was in a local
laundry mat recently, as he was placing his laundry in the dryer a ceiling fan apparently came
down still attached to the wire and spinning and struck the front of the patients forehead and the
back of his head knocking him down to the ground, as he got up he was struck a third time, this
time in the neck region. He denies any loss of consciousness. He states that the ceiling fan
dropped down a proximally 3 to 4 feet. He notified the owner of the laundry mat as well as the
police and a report was filed. He states that the next day he went to the emergency department
