SUBJECTIVE: Stavros Zalaris, 33 years old Greek male, presented send follow up for his
history for primary hypothyroidism. And thyroid hormones of the resistance of uncertain
etiology. Patient is now on 175 Micrograms of Levoxyl and feels well. He has no recognized
symptoms of thyroid deficiency or excess. Despite it is high normal TSH in past, he has not
tolerated higher replacement do with symptoms of heat intolerance. Although he still sweat
easily, he feels overall is doing well; he has no particular desire to make any changes of this
dosing. Internal history was discussed. Medications were reviewed. He has decreased the
CSA and may be stopping Plavix due to easy bruising.
OBJECTIVE: VITAL SIGN: PULSE 78 irrgular, BP right arm sitting:111/78, 120/74.
HEENT: EYES: without led light. There is no tremor. Iris flexes and sclera normal. NECK:
