TURN OFF YOUR CELL PHONES AND PUT THEM AWAY! If you are wearing a hat and haven’t already done so, please point the bill backward. Then, carefully follow these instructions. 1. There are 60 questions on this test. Check your booklet now to see if it has all 60 questions. Then print your name legibly above left and sign it above right. 2. Use only a No. 2 lead pencil. 3. On the scantron sheet, record your name, U.F. Student ID number and form code , and bubble them in completely. Double check that all information is correct and legible. IF YOU DO NOT BUBBLE IN YOUR UFID CORRECTLY, YOU WILL BE UNABLE TO RETRIEVE YOUR SCORE ON THE COURSE WEBSITE. 4. Your section number is not required. 5. Record all your answers on your computer (SCANTRON) scoring sheet. Be sure to double-check your scantron; errors in “bubbling” your answers are your responsibility. 6. Mark the best answer to each question. 7. When you have completed the exam and double-checked your answers, take your exam booklet and SCANTRON sheet to a proctor at the front of the room. YOU MUST SHOW YOUR EXAM BOOKLET AND YOUR PICTURE I.D. WHEN YOU TURN IN YOUR SCANTRON. You must wait until the proctor has identified you before leaving. 8. Penalties: You will be penalized FIVE POINTS if you are in the wrong room. You will receive a zero on this test if you use a graphing calculator, cell phone or PDA, or if you do not promptly and courteously comply with a proctor’s request. To avoid any suspicion of cheating, please do not talk with your neighbors for any reason. If you have a question, raise your hand and a proctor will help you. 9. Proctors are not allowed to clarify any questions, give you the definitions of words, etc. If there is a problem with any question (e.g., a serious typographical error), I will take care of it as part of the appeals process. 10. Grades will be posted on the course website as soon as they are available. Because of the appeals process and the possible re-scoring of the exam, this process takes several days. Please be patient. MAR 3023 MIDTERM 1 FALL 2010 FORM B The exam answer key will be posted on the course website tonight by 10:30 p.m. and emailed to the course email list. You may appeal no more than two questions . Appeals must be addressed to <[email protected]>. Important : Identify the Form code and question number in the subject line of the email, and provide a page number from the text or notes to support your appeal. Unsupported appeals will not be considered. One question per email, please , to facilitate sorting; multiple appeals in one email will be discarded.
