TEST FORM TEST FORM Fall 2010 MIS 304 – In-Class Exam 1 Page 1 of 10 A Time allowed: 60 minutes Exam questions: 100 points (Questions 1-32 , each worth 3 points + 4 pts for correctly completed Scantron form) Remember : This exam is closed book , closed notes , closed electronics (this includes laptops, cellular phones, PDAs, Blackberry’s, iPods or any other electronic device), or any other personal item, such as book bags or purses . You may NOT use a calculator or dictionary during this exam for any reason. After the exam begins, if you have to leave the classroom for any reason (bathroom break, emergency phone call, illness, etc), your must turn in your exam before you leave the classroom and you will not be allowed to continue your exam afterward. In you have comment or concerns of a general nature regarding the exam, please write your comments on the back of the Scantron form and contact your instructor after the exam via email. Instructions : On the left side of the Scantron form, complete the following sections: ID NUMBER (Student ID) and NAME. YOUR ID NUMBER = THE LAST 9 DIGITS OF YOUR CATCARD NUMBER!!! In the “PHONE NUMBER” section enter the first six digits as “111 00x”, where “x” is your section number! You must mark all answers on your Scantron form with a #2 (HB soft lead) pencil. When correcting an error on your Scantron form, be sure that your incorrect answers are erased completely. Incorrectly marked questions (ones with multiple answers for the same question or mark that cannot be scanned) will be scored as incorrect. Do not make any stray marks on the Scantron card, as this may be read as a response. All questions will be graded, so you should mark an answer for all questions. You must turn in both the Scantron form and your copy of the exam booklet.
