Focused Exam: Chest Pain

CC: "Chest pain now and then for the past month"
HPI: 58-year-old Caucasian male presents to the clinic today with c/o intermittent moderate chest pain
that started earlier this month. He described it as a "tightness" in the substernal area that he correlates
to occurring with physical activity and resolves with rest. He denies radiation of the pain, dyspnea, or
any dizziness. Pt denies any past cardiac history, reports he has had several EKG tests done in the
past, and has had annual "stress tests" which he believes to have been normal. Patient is currently pain
free but rates the chest pain at 5/10 when it does occur.
Medications: Metoprolol 100mg daily Atorvastatin 20mg daily Omega 3 fish oil 1200mg twice daily
Allergies: Codeine - N/V
PMH: HTN, hyperlipidemia, no surgeries. UTD on Tdap and influenza vaccines
Social History: Employed as an engineer, 16 years at his current place of business. Married for 27
years with 2 adult children. Never smoker, pt. does report "a few beers" on the weekends
