Unformatted text preview: Exam Code: A Name (Printed) ____________________________________ Signature ____________________________________ SS# ______________________ INSTRUCTIONS READ THIS PAGE CAREFULLY BEFORE YOU BEGIN YOU MUST BE IN THE CORRECT ROOM OR YOUR EXAM WILL NOT BE GRADED DO NOT OPEN THIS EXAM UNTIL INSTRUCTED TO DO SO 1. Close all books. The only items allowed on the desk are a #2 pencil, your exam, your scantron answer sheet, your 1-page cheat sheet (8 1/2 x11, plain, white typing paper only), and your calculator. All other materials must be placed on the floor under your chair and backpacks, purses, etc., must be closed. 2. Print your name on this exam, fill in your SS#, and then sign this exam in the spaces provided. 3. Fill in the information requested on the scantron answer sheet and then bubble in the corresponding data: Last Name, First Initial, Middle Initial, SS#. Do not fill in a Section number. 4. Verify that the Test Form Code on the scantron answer sheet matches the exam code in the upper right-hand corner of this exam. 5. Ask a proctor for help if the codes do not match. Do not attempt to erase or alter the Test Form Code (they are pre-bubbled and there are multiple security codes for matching the correct test form code). Doing so will invalidate your exam. 6. This exam consists of 20 questions and each question is worth 1 point. Answer the questions in Rows 1-20 of the scantron. Select the most correct answer. If multiple answers are correct, choose the answer that indicates this. 7. Points will not be deducted for incorrect answers, so you should attempt to answer all of the questions. 8. Proctors will not answer questions involving the exam materials or questions on the exam: being able to understand the question is part of the exam. 9. Proctors may move students during the exam to lessen the possibility for cheating. If you are asked to move, it does not mean that you are cheating: you may be moved so that another student can not copy from your exam. 10. Cheating is a serious matter and will be dealt with severely. By turning in the scantron answer sheet to this exam you are confirming that all work on this exam is your own and that you have not received help from any other source (i.e., cheated) in answering the questions on this exam. 11. You must use your time wisely. Do not spend too much time on any one problem. If you get stuck, leave the problem and then come back to it if you have time. 12. Bubble in your answers as you go. You will not be granted extra time to bubble in your scantron at the end of the exam. Your exam will not be graded if you do not immediately stop working on the exam when time is called. 13. Students who finish their exam early and wish to leave the room during the first 1 1/2 hours will be asked to turn in their exam along with their scantron answer sheet....
