Health Assessment The assessment not only requires physical data but an  integration of biological, psychosocial, and functional aspects  of the person. Assessment of older adult requires special abilities of the  nurse: Listen patiently Allow for pauses Ask questions that are often not asked Observe minute details Obtain data from all available sources Recognize normal changes associated with late life that might  be considered abnormal in one who is younger Nursing assessment of older adult takes longer because of  the increased medical and social complexities of older adults. Initiation of health history marks the beginning of the nurse- patient relationship and the assessment process.

Health History Patient profile Past medical history Review of symptoms and systems Medication history to include prescribed  medications, OTC medications, and herbals and  dietary supplements Family history especially parents and siblings Social history to include current living  arrangements, economic resources, amount of  family and friend support, and types of community  resources available Health history should also include the identification  of those who are involved in health care decision  making
