How can a community health nurse contribute to healthier population? What tools do
they need to analyze and make changes? I plan to use the platform of this paper to show you how
community health nurses’ function as community wellness advocates and what assessments they
use to collect their data. To complete my task, I utilized a computer simulation called Sentinel
City. Sentinel City is a virtual community, that is comprised of four districts, and a multitude of
simulated hot spots to visit. For this paper, I will be reviewing tools to assess and summarize the
health needs for any given community. These tools include demographic assessment, community
safety inventory, population health scavenger hunt, and a windshield survey. Next, I will
complete a diagnosis for Sentinel city utilizing the Health People 2020 Goals and discuss what
resources are available to help address the problems identified. Lastly, I will take my newfound
knowledge of assessments and apply it to my hometown community. All information regarding
Sentinel City was obtained directly through the simulation available through the WGU website.
Needs Assessment Summary
Demographic Assessment
The first tool I utilized was the demographic assessment tool. The combined population
of Sentinel city’s four districts is 663,862 (Healthcare Learning Innovations, 2018). The
population can then be broken down into four subcategories or districts. The district with the
highest population density is Casper Park District, with a population of 352,643, sixty-three
percent white and twenty-four percent latino (Healthcare Learning Innovations, 2018). The next
highest population is Acer Tech Center at 168,390 people, ethnically divided as seventy percent
white and thirteen percent latino (Healthcare Learning Innovations, 2018). Nightingale Square
consists of 103,974 people, coming in as the third most populated district (Healthcare Learning
Innovations, 2018). Nightingale Square can be further divided ethnically with eighty-one percent

being white and three-point seven percent latino people (Healthcare Learning Innovations,
2018). The last district is Industrial Heights with a population of 38,855 and culturally divided as
forty-six percent latino and thirteen percent African American.
Sentinel City’s collective ethnicity is another component I was able collect information
on. Caucasian (non-Hispanic) citizens comprises of 52.7 percent of the population (Healthcare
Learning Innovations, 2018). The next largest ethnic group is the Latinos at 31.5 percent
(Healthcare Learning Innovations, 2018). African Americans complete the third largest group at
10.4 percent of the population (Healthcare Learning Innovations, 2018). Asian ethnicity
encompasses 3.7 percent of the population (Healthcare Learning Innovations, 2018). Citizens
who identify a s two or more races include 3.1 percent of the city’s population (Healthcare
Learning Innovations, 2018). American Indian or Alaska Natives comprise 2.0 percent of
Sentinel city’s population (Healthcare Learning Innovations, 2018). The last population group at
