Lecture 9 Weight Control  Weight Control  Gastric Bypass  3 Types Restrictive  limits food intake by creating narrow passage from upper part of stomach  into the larger lower part Malabsorptive  excludes most of the SI No longer Rx b/c results in severe nutritional def Combined restrictive/malabsorptive  Retrictive Gastic bypass  Limits food intake Does not interfere with normal digestive process Small pouch is created at top of stomach Pouch holds   ~1 oz , and later may stretch to: 2-3 oz  Cannot eat large amounts of fd at one time Can eat ~1/2 – 1 cup fd w/o discomfort or nausea

Restrictive Gastric Bypass  Adjustable gastric banding (AGB)  Hallow band made of silicone rubber placed around the stomach, near its upper  end Band is inflated w/salt solution through a tube that connects the band to an  access port placed under skin Can be tightened or loosened over time to change size of the passage by  increase or Decrease    amount of salt solution  Restrictive Gastric Bypass-AGB  ADVANTAGES OF AGB: Easier to perform Safer than: malabsorptive damage  Laparoscopy: small incisions,  Shorter operation time & hospital stay (less cost) - Less risky  Restrictive Gastric Bypass-AGB  DISADVANTAGES OF AGB: Lose less weight than malabsorptive operations

Less likely to maintain wt loss over the long term Lose ~1/2 of   excess wt in 1 st  year After 3-5yrs,  regain some of weight loss  After 10yrs, 20% have kept weight off  Restrictive Gastric Bypass-AGB  RISKS OF AGB: Vomiting  when you eat too much Slippage  or Wearing away of band Salt solution leaks Requiring another operation to repair  Infections. Bleeding potential exists
